View Waiver
By signing below, I acknowledge and agree to the following:
I give permission for my child to participate in Vacation Bible School activities hosted by Calvary Baptist Church. I understand that participation in church activities may involve certain inherent risks, including recreational activities, group events, and transportation.
I give permission for my child to be transported by authorized staff members or approved volunteers of Calvary Baptist Church in church-owned or privately owned vehicles for purposes related to Vacation Bible School activities.
I authorize emergency medical treatment for my child in the event of an emergency if I cannot be reached immediately. I understand that reasonable efforts will be made to contact me as soon as possible.
In consideration of my child being allowed to participate in these activities, I release and hold harmless Calvary Baptist Church, its pastors, staff members, volunteers, and representatives from liability for any injury, illness, accident, loss, or damage that may occur during participation in Vacation Bible School activities or transportation to and from those activities, except in cases of gross negligence or willful misconduct.
I also understand that photos or videos may be taken during Vacation Bible School activities and may be used by Calvary Baptist Church for ministry-related purposes including social media, promotional materials, and church communications.
By signing below, I acknowledge that I have read and understand this waiver and voluntarily agree to its terms.
